COMSERV offers three (3) different DIS investigation report formats from which to choose. The DIS investigation results may be printed in the selected report format with records sorted in any order at no additional cost. By default, Report Format 1 is used to print the DIS investigation report in social security number (SSN) order. If any DIS report format does not meet your requirements, simply inform us of your report format preference.
